Comprehensive individual assessment
The first step in the care management process is to determine the needs of the client. This is accomplished by performing a comprehensive individual assessment that will consist of a broad range of tests and interviews, including the examination of one’s mental health and cognitive function, functional status, nutritional status, medication therapy and the entire medical treatment plan of care. An in-depth review of the client’s psychosocial history will also be conducted. Home safety assessments are important
In addition to an Individual Assessment, Elder Care Consultants performs a Home Safety Assessment. This consists of a thorough safety assessment of the client’s residential setting. Home safety assessments will identify areas potentially placing the elder at risk that include scattered rugs, steep or narrow steps, non-handicap accessible entryways or bathrooms, and various fire and environmental hazards.
